FHP Home Health, a Medicare certified agency, gives you the option to receive medical care at home as recommended by your doctor.

 


 

Whether you opt to avoid a hospital stay or come home from the hospital sooner, prefer the comfort of home or choose to save funds, FHP Home Health replicates the same advanced treatment and quality care as the Medical Center.
 

FHP Home Health is also a Medicare certified Hospice provider, committed to providing comfort and expert care for patients with terminal illnesses. Our hospice care is a special unit designed to give patients and their families all the support and medical guidance they need – it’s hospital care right in the comfort of their home surrounded by family.
 

Our Home Health providers belong to a team of compassionate people with exacting professional commitment. They include registered nurses, a licensed practical nurse, respiratory therapists, physical and occupational therapists, and social workers.

About Hospice Care

The services provided by our Hospice programs are palliative and not curative.  The program emphasizes the management of pain and physical symptoms as well as meeting the emotional, social and spiritual needs faced during terminal illness.The services of FHP Home Health and Hospice are delivered in the home by a team of Hospice professionals.  These services include nursing, social work services, home health aides, therapy services and bereavement counseling. The hospice team is not intended to take the place of family, but rather to support the family in caring for the patient and in mobilizing available resources.  In order to qualify for Hospice services, a patient must have a terminal diagnosis and must not be seeking curative health care.  Your hospice 


Care team will make a visit to the home to meet with the patient and family to explain the program and answer any questions.  This will be scheduled weeks before the patient is ready to transition into hospice care.

 




Bereavement Program

The period right after the death of a loved one is a time of regrouping and sometimes relief.  One should not feel guilty about the sense of relief.  More active grieving and sadness may occur later.  The members of our FHP Hospice team will be available to help you through this difficult time.Hospice is available to help family members and significant others to continue to live theireveryday lives after the patient’s death.  Members of our FHP Hospice Team will continue to support you by telephone and mail.  Several support groups are made available to the family through our Hospice.

How to start Home Care or Hospice Care

A referral must be obtained from a physician.The patient must be home bound to qualify for services.patient and family.Our Home Health team will provide the services.

 




Explanation of Services


Skilled Nursing
Provides skilled nursing care to include wound care, IV therapy, medication management, disease management, rehabilitation nursing, Foley care and ostomy care.

 

Physical Therapy
Provides Physical Therapy services to include gait training, cardiac rehabilitation, transfer techniques, wound therapy, assessment or range of motion.

 

Home Health Aide
Personal care services to include bathing, dressing, toileting, transferring.  The aid assists with use of bedpan, beside commode and use of the bathroom.  Aides also provide skin care and lotion application.  The aides perform range of motion exercises under the direction of a nurse or therapist.

 

Social Work Services for TakeCare
Assistance and counseling for the patient and family on areas that impact health care.
 

Hospice Services
Offers palliative care and treatment for patients that have a terminal diagnosis.

 


 

Services Covered

Medicare recipients have 100% coverage for Home Health Services.  No co-pay is required.  Patients who have other private insurance should consult with their carrier to determine coverage amounts and co-pays.
